Introducing GUIDE: How Synapticure is Revolutionizing Dementia Care for Patients and Caregivers
Living with dementia can be challenging, not only for individuals diagnosed but also for their caregivers. The Guiding an Improved Dementia Experience (GUIDE) Program is a new Medicare initiative designed to provide expert dementia care, reduce caregiver stress, and help individuals with dementia stay safely in their homes—all at no extra cost for eligible beneficiaries.

What Does the GUIDE Program Provide?
Personalized Care Plans & Medication Management

Dementia care requires continuous support. Through the GUIDE program, Synapticure offers:
- A 24/7 helpline for urgent medical concerns or general questions.
- Care navigation services to help with medication delivery, meal services, home safety assessments, and connecting with local resources.
- Ongoing support to help you adapt to new challenges as dementia progresses.
Caregiver Support & Respite Care
Caregiving can be overwhelming, but you don’t have to do it alone. The GUIDE program provides up to 80 hours per year of respite care, giving unpaid caregivers a chance to rest while their loved ones remain in safe hands.
Synapticure also offers caregivers education and skills training to help families better understand dementia, manage daily care, and plan for the future. Our team assists with coordinating local services, such as transportation, home modifications, and meal deliveries, so caregivers can focus on what matters most.
Who is Eligible?
To qualify for GUIDE, individuals must:
- Be enrolled in Traditional Medicare (Part A and B)
- Have a suspected or confirmed dementia diagnosis
- Live at home or in the community (not in long-term care)
- Not be currently enrolled in hospice or PACE programs
If you don’t qualify for GUIDE, Synapticure still provides ongoing dementia care and caregiver support. We are happy to discuss your options and find the best solution for you.
